How to have perfect breasts with breast augmentation

Whether pin-up, as big as a champagne glass or teardrop-shaped, breasts are a fundamental weapon of seduction and the desire to have them perfect pushes more and more women to have surgery to improve them. There breast augmentation it is, in fact, the most requested plastic surgery procedure in the world, with over 55.000 breast augmentation procedures per year in Italy alone.

Despite being an extremely requested procedure, it remains a very complex operation in its simplicity: every woman has different, unique breasts, and each result will be different, exceptional based on the starting shape. No natural breast is the same as another, just as no operated breast will be the same as another.

For this reason, before undertaking a breast augmentation it is necessary to undergo a thorough examination, so that the plastic surgeon can evaluate the starting point of the breasts, the structure of the chest, determine whether there are malformations or complexities such as tubular breasts , tuberous, breast apoplasia, and promptly plan the operation most suited to the type of breast.

In this way, an additive mammoplasty is not reduced to a simple insertion of prostheses but becomes a real sculpture, a design of the breast studied in every detail on the patient's body, in order to choose the most suitable technique or, possibly, combine them different together.

In addition to the patient's starting point, in fact, the techniques for positioning the prostheses affect the aesthetic result and the design of the perfect breast will have to take into account the most suitable procedure.

The placement of breast implants can be done in 3 ways: it can be glandular back, then behind the mammary gland, muscular back, behind the muscle, o dual plane, partially retro muscular. With technique dual plane, based on the type of detachment (type I, II, III) it is possible to correct mild breast ptosis without resorting to mastopexy and without leaving obvious scars.

If the sagging skin is more severe (breast ptosis can be of varying degrees of severity) it is necessary to resort to a mastopexy, an operation aimed at correcting breast ptosis, returning it to its original position. In this case we are no longer talking about standard breast augmentation but about breast correction, which can also take place with the aid of prostheses or through a reduction mammoplasty in the case of extremely large breasts.

Prostheses, an element whose choice is fundamental for the natural result of a breast augmentation, are of different types and various sizes and shapes and it is not always easy to orient yourself. Let's try to clarify things a little.

Le round prostheses, the first introduced on the market, have a semi-spherical shape and a central, fixed projection. The gel inside them has different degrees of cohesiveness (understood as the density of the gel) and the different degrees of density correspond to the greater or lesser softness of the prosthesis, a fundamental characteristic in planning an operation.

Furthermore, these prostheses have different textures: they can be smooth, micro-textured and textured or macro-textured. Once both smooth and textured have been used, following the discovery of a presumed link between textured prostheses and the appearance, in very rare cases, of Bia-Alcl, a large cell anaplastic lymphoma tumor, there is a tendency to use only smooth or textured prostheses. microtextured.

Le anatomical prostheses they are drop-shaped and have maximum projection in the lower pole of the breast: the volume is concentrated below the nipple to try to give a natural effect. They are usually textured in order to create an "anchoring" to the inframammary tissues and avoid their rotation, an unfortunately quite frequent complication. In my opinion they are not suitable for all patients, precisely because of the risk of complications, and they are not particularly soft to the touch, remaining quite unnatural.

In recent years, the B Lite, anatomical prostheses whose content is composed of silicone gel to which borosilicate microspheres have been added. This allows them to be very light (they weigh about 30% less than other prostheses) but the texture is very hard and compact.

The most recent prostheses are the ergonomic, in the shape of a semi-sphere like the roundabouts but with the point of maximum projection variable. Thanks to the extremely soft silicone structure, they are able to dynamically adapt to the body's movements, effectively simulating the projection of anatomical prostheses but avoiding complications. These prostheses guarantee very soft breasts and an optimal aesthetic result.
To date, in the presence of the right conditions, they are the prostheses I prefer to implant.

Still regarding breast augmentation, in recent years there has been a lot of talk about lipophiling, that is, the possibility of filling the breasts with fat taken from one's own body to increase their volume. Personally I advise against it, believing that the result is not so satisfactory as to choose it as my favorite technique.

On the contrary, I am a lover of combined breast augmentations, in which I use fat to perfect the procedure. In this way, it is possible to sculpt or reshape asymmetries that cannot be improved with the insertion of prostheses alone; fix the depressions in the sternum (pectum incavatus) which often compromise the result of an augmentation or, in the case of very thin patients, improve the thickness of the tissues to make the prostheses invisible.

Breast augmentation surgery – Post-surgery scar

The choice of access for the insertion of prostheses is another aspect to be evaluated from the perspective of a perfect and natural breast, to have small and well hidden scars. Usually the access of the prostheses can be from axillary pillar, peri areolar (with access to half moon in the nipple) or from breast fold.

The technique I prefer to use is a small hidden access in the groove of the breast fold, in some cases no more than 2 cm, so that the scar can become invisible over time, thus reducing any sign of breast augmentation to give my patients breasts. attention to every detail.

In addition to the technical aspects, which are fundamental in planning the intervention, even more important is the choice of the professional to rely on. It is essential to opt for a skilled and trained reconstructive and aesthetic plastic surgeon, with experience and a sufficient history of interventions to be able to guarantee valid and long-lasting results. A poorly performed breast augmentation surgery could be very difficult to correct later, which is why it is necessary to rely on expert hands.
